A Disability Support Worker is responsible for helping clients and customers enjoy independent living. This can be in a residential services or requires travelling between customer’s homes to assist them with daily living and lifestyle requirements by providing person-centred care and support.

 

What you will typically do in your role:

• Personal care: showering, dressing, grooming and medication assistance

• Domestic assistance: light household cleaning, laundry and meal preparation

• Social support/transportation: social activities, shopping/groceries, travelling to medical appointments

• Supporting customers in the use of aids and personal equipment including: standing or walking frames, wheelchairs, manual or electric hoists, sliding sheets or moving boards, hearing aids and other physical aids

• Supporting customers with complex and/or challenging needs

• Monitoring the well-being of customers and reporting on their progress

• Participating in staff meetings and paid training as required

 

Typical requirements for these roles:

• Valid CPR / First Aid Certificate 

• NDIS workers screening check  

• Minimum Certificate III or IV in Aged Care, Home & Community Care, Individual Support or Disability Services (preferred)

The Care or Case Manager enhances the quality of the client’s or customer life and is responsible for assigned clients. The roles assess, plans, implements, monitors and evaluates actions required to meet the client’s health and human services needs.

 

What you will typically do in your role:

• Deliver a holistic approach to person centred care, clinical and or nursing care care, focusing on the needs and rights of clients; 

• Provide comprehensive, safe and effective evidence-based care to achieve identified individual health outcomes;

• Incident reporting; 

• Review potential consumers; 

• Review & update each consumer’s care plan; 

• Receive and conduction education & training; 

• Participate in clinical meetings as required; 

• Ensure compliance, policies, and procedures, are implemented and enforced 

• Support the competence and accountability of team leader. 

 

Typical requirements for these roles:


•Minimum Diploma qualification in Community Services or related field – ideally a relevant degree
• Minimum 1 year of experience in Disability and or Aged Care services
• WWCC

The Support Coordinator role exists to connect customer / participants with an NDIS plan. Generally the Support Coordinator is the first point of contact for NDIS participants, families to coordinate and provide services under an NDIS plan.

 

What you will typically do in your role:

• Research, coordinate and manage a range of supports to suit individual needs

• Coordinate access to support items in an NDIS Participant’s Plan in line with the preferences of the participant to achieve the outcomes identified in the plan

• Support participants to strengthen their ability to connect with informal, mainstream and funded supports, and to increase capacity to maintain support relationships, and resolve service delivery issues within their local communities

• Support individuals to build their capacity and networks with the aim of greater choice, control and independence in managing their personal lifestyle choices

• Ensure that service provision is informed by the participants’ individual needs and is in line with current participant protocols or individual service plans

 

Typical requirements for these roles:

 

• Minimum Certificate IV in Disability or related field

• Demonstrated knowledge and experience of NDIS

• Strong interpersonal, written, and verbal communication skills

• Ability to develop good working relationships with internal and external stakeholders, including participants, families, and health providers.

• Effective time management, caseload management, problem-solving skills, and ability to plan and manage resources/budgets

• Knowledge and understanding of the NDIS, and ability to work within their framework.

• Experience in working with people with complex support and service requirements.

Behaviour Support Practitionersiconmonstr-arrow-right-thin-svg

As a Behaviour Support Practitioner you will be required to conduct functional behaviour assessments to develop and implement behaviour support plans for clients / customers. This will involve interviewing clients and their support networks, conducting observations, analysing data, administering clinical assessments (within professional scope), developing & implementing strategies to engage people in meaningful activities, address quality of life issues, as well as implement risk assessment measures to keep people safe and free from harm. This role involves can be based in a residnetial service or working with clients in the community in their own home. So some travel is required to deliver behaviour support in home, at school and other community places. Often these roles can be a mixed caseload role, working with both adults and children.

 

What you will typically do in your role:

 

• Support adults and children with behaviour support plans.

• Helping adults and children achieve their goals by reducing their behaviours of concern and improving their quality of life

• Contributing to the ongoing review of behaviour support systems and the provision of behaviour support in the NDIS context

• Conducting behaviour assessments and developing written behaviour support plans for complex behaviour referrals

• Positive Behaviour Support, writing positive behaviour support plans

• Working in a collaborative trans-disciplinary team of therapists

• Conduct staff training in individual behaviour support plans and strategies

 

Typical requirements for these roles:

 

• Tertiary qulaification in Allied Health or Applied Behaviour Analysis.

• NDIS registration

• Working knowledge of positive behaviour support practices, relevant legislation in the State

• Excellent interpersonal communication and negotiation skills, with the ability to build rapport

A disability team leader oversees a disability support team in an effort to provide care and assistance to those needing help living their everyday lives. Disability team leaders delegate responsibilities, monitor patient or client intake and assessment and determine how to best provide aid. Often overseeing rosters and day to day operations of either a day programn, residential services or Customer Engagement/Case Management Team.

What you will typically do in your role:

 

You will be efficiently and effectively managing your team and the teams resources to ensure the delivery of quality services to clients/customers. This entails having fantastic communication skills and building working relationships with internal and external stakeholders. You need to have the ability to report and process complaints and critical client incidences.

 

Typical requirements for these roles:

• Tertiary qualifications or relevant vocational experience in management or leadership.

• Demonstrated ability to deliver quality service in a dynamic and fast paced business environment.

• Previous experience working in the disability sector

• Knowledge of disability and human services standards.

A Clinical Specialist working within disability services could mean you have had prior training and experience in Nursing, Psychology or Counseling or similar and experience often working in multi-disciplinary teams. 

 

Typical requirements for these roles:

 

• A minimum of a Bachelor’s degree in Social Work, Psychology and/or Behavioural or Social Science, nursing and or and counselling.

• A minimum of 2 years’ demonstrated experience in the provision of trauma informed practice model counselling and clinical support services to individuals who have experienced trauma, abuse, neglect and or complex support needs.

• Demonstrated understanding of cross-cultural issues in providing services to individuals, families and communities 

• Capacity to engage in reflective practice and take responsibility for proactive self-care

• Demonstrated ability to provide education and information sessions to community members

The role of the Program Manager is to provide leadership, coordination, engagement and program management support.

 

Typical requirements for these roles:

• Relevant tertiary qualification in behavioural sciences and/or relevant experience

• Demonstrated ability to provide leadership of a team of staff including supervision and mentoring skills

• Significant experience in program management preferably in a health, local government and/or community services environment.

• High level written and verbal communication skills, including administrative experience in using office computer systems, databases and spreadsheets

• Understanding of cultural competent responses when engaging with Aboriginal children, families and communities

• Sound understanding of the primary health care system and local community services within the context of integrated models of care.

• Demonstrable ability to foster and maintain strong partnerships with disparate internal and external stakeholders including senior executives, government representatives, local population health/health planners, clinicians, primary care providers

Area Managers or Service Managers often can have smaller or larger portfolio’s depending on the service provision. These position will provide strong leadership and motivation to staff whilst effectively managing operational management. They require a a sound understanding of the current trends and external environment to inform strategic direction. Overseeing Program Delivery, growth and planning, quality and compliance.

Typical requirements for these roles:

• Demonstrated experience working within disability services either across adult and or children services, including the management of staff within this environment

• Excellent communication and interpersonal skills, with the ability to liaise with a wide range of individuals including clients, staff, government departments and other stakeholders.

• Strong client focus and the ability to establish and maintain relationships with a range of participants or clients

• Experience working with a range of people with a disability in a range of settings.

• Working knowledge and understanding of disability specific legislation, the NDIS Quality and Safeguards Commission, NDIS Practice Standards and code of Conduct for workers and providers and other relevant legislation, guidelines, policies and trends in service provision to people with a disability and implications for service delivery.

• Passion for supporting and coaching staff, managing, and developing a team to achieve outcomes.

• Demonstrate technical knowledge and understanding of NDIS Price Guide and Participant Plan funding.
